Selected Attribute Requires Text

Is there a way to setup an attribute on a product that when it's selected, a text field that requires input appears?

Yes, the option type TEXT does that.

I've get the text field to show, but cant seem to figure out how to make it reliant on a check box.

That sounds like "dependent attributes", which are not supported in current Zen Cart. You would have to custom code the javascript to do what you want.
