Custom shipping module instructions/tutorial/template/framework

Hello, I'm using v 1.3.7.1. I would like to write my own custom shipping module to meet unique requirements. Are there instructions and/or skeletal code or a template/framework for doing so? I just need to know the available variables from the cart, and what values need to be returned!

I've looked at the code in sample modules, and can extract bits and pieces of what I'm looking for, but I'd really like unified source material by way of a guide with all interface requirements and variables/objects.

Check the flat clone mod, there is a guide that shows how to create others. Hope it helps.
