Reviews on Site cant be enabled

Hi,

I'm having a problem with Zencart 1.3.8. A customer posted a review on an item, but when I go to approve it by clicking on status it wont enable and just stays red. Also, when i click on detail it shows the page but everything all the fields are empty. The images seem broken they're pointing toward /includes/templates/template_default/images/

Any ideas on how to fix this?
