Any one know how to set up the form or have an example to view?
Sure would help.
Or is there any other way to buy more than one item at a time instead of going to each page and hitting add to cart?

Hi, go into Admin,
In Menu Configuration->Product Listing set
Display Product Add to Cart Button (0=off; 1=on; 2=on with Qty Box per Product) set value 1
and
Display Multiple Products Qty Box Status and Set Button Location set value 3

This way on products listintings page under each products apareas a quantity box and one Add to cart button on page top and bottom
