I just had an odd transaction.
Customer attempted to place a credit card order and clicked the confirm button. Customer says they then got a screen indicating "the page can't be displayed".
Authorize.net shows tentative order #38168-xxxx and captured the payment from the customer's first (failed) attempt to place the order.
In the meantime, a different customer placed order 38168 and their charge and order came through fine.
Going back to the customer with the problem, they "claim" that they hit "back" on their browser after encountering the error page. They said that they checked their payment info to make sure it was correct, and checked out again. This time I got a payment with tentative order #38169-xxxx, which matches the order they placed.
Both payments from this person each equal the full amount of the order, so authorize.net processed their payment twice, but I only got the order once.
To summarize, customer was able to pay 2 times for 1 order and I got no indication that occurred except that I got 2 merchant email receipts (which I almost didn't notice). Same customer id, card number, and order amount on both transactions.
I didn't think this was possible so I figured I ought to report it.
