NMI addon problem

I'm using 1.3.9h and I installed the NMI add-on and I'm having a blank page after someone tries to check out.

I already looked at the blank page FAQ and I can't seem to figure out where the debug is already installed and run it. I made sure the cache folder was writeable but nothing is getting up there.

I figured out this part, I had to find the keys on their end...

Now though, still a blank page and the url says this:

checkout_process&response=2&responsetext=INVALID+STORE++++++++++++++++++++.

Ok they fixed it, it was on their end but I'm still getting a blank page now.

The transaction goes through, I see it as orders placed in the admin panel but I'm not getting emails notifying me that an order has gone through.

I've gone through the blank page FAQ already and reuploaded everything twice.

Any insights?
