optional insurance mod

Hi i was told to use this mod

basically what i'm after is a mod that will add £1 to their order
but instead of it being for insurance i want to change the wording to read 'optional donation' that is ticked as standard but can be untick if the customer doesn't want to add it.

how do i set the settings pls and what do i have to change to get the wording i want

I haven't used the mod, so I don't know if the checkbox is possible.

also does anyone know how to do the settings so it just adds
£1 rather than a percentage of the order
