Customer Notified - Error After Updating

This feature worked at one point with out any issues. My wife just brought it to my attention but she said it has been happening for some time now.

If a customer orders a leaves a comment there is a green check next to that box. The next box is either 1 or 2 notifications from PayPal. Those are both marked with red X's. I understand that Paypal already sends a confirmation e-mail so i dont need to have one sent.

When we provide the tracking # for the customers order we select update, put the tracking # and some other text in the comment box and click update. I then get a HTTP 500 Internal Server Error message. I tested this with a dummy order. I still get the update but why am I seeing the error after clicking update?

When i go back to view the comment there is a green check.
