Attribute Controller not loading

As stated, I had not changed all the files, only the two above.

First, can you explain this, as there seems to be a difference in interface

NO, first provide the two full file names and the path that you have changed, and what you changed

From admin\includes\header.php, removed the Zencart <IMG> from the top.

Ok, That 3rd party module should have no effect on your issue

A stock install does not exhibit what the issue you have posted about

Suggest that you check that you replaced the changed files and also check for the removed files

I am also having this issue with 1.3.9h where I get bumped out to the admin home page when I try to insert an option in the Attributes Controller. I have added many modules and customized many files to this site. And perhaps it isn't even a problem with a modified file - but instead with a file that was corrupted during upload.

Can you tell me where to look, in general, to re-upload files to repair this. Such as, "probably the problem file is in admin/includes". Or something like that, so I can narrow my search down?

Just to make sure you understand: I am not asking for you to diagnose my problem. I am asking what file affect the Attributes Controller and are "likely suspects". I could then start which checking, re-uploading (or merging) those. Thanks!
