gjh42, I changed the name of my images to paisley and paisley_01 because I could not see what you were pointing out.
Are you saying that you can see my one additional image on the product info page?? I rebooted my machine just to make sure. I do NOT see the additional image?

I can confirm that the additional image does not appear when using IE8 or IE7 Compatibility View. It works correctly in Firefox.
This probably means that there is some error that IE is not able to handle; however, validators show no HTML or CSS errors.
It appears that some javascript is malfunctioning, as when I enable js in Firefox, the additional image disappears. The "Simple Image Trail script- By JavaScriptKit.com" is most likely the culprit.
NOTE: ... I cannot see the additional image in FireFox ...

That's because you are allowing javascript. The script on the additional image is malfunctioning somehow.
But the additional image was not showing before I added IH2. I only added that because I found another poster who had the same prob and this somehow "fixed" it for him.
I can't see the additional image in Firefox either.
How does one fix this? I am only using a simple template.

It is a javascript malfunction of some sort that is causing your problem. I can't tell what the problem is or where it came from, but with js disabled, the additional image works fine.
