adding forms

hi all,

I have a need to add a few surveys to my site - from that, what is the best way to add forms to a zen cart shop?

Do i have to add them via code, or is there an add-on that allows me to easily construct a form which then sends the responses via email or notifications?

BTW I'm using 1.3.9c
