attributes error

Hi

I am having a problem trying to add attributes
I can add Product Options no problem but when I go across to Option Values I get this error

1062 Duplicate entry '1-1' for key 1
in:
[insert into zen_products_options_values (products_options_values_id, language_id, products_options_values_name, products_options_values_sort_order) values ('1', '1', 'product', '1')]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.

and can go no further

I am using latest version ,win 7 , using firefox 3.6.16
Do have a few add-on but don't think any of them interfere with attributes ..... I did copy across the add on attributes but that didn't work either ,so returned back to normal

not 100% sure what is wrong but have done some searching but so far none of them worked ...

got this message back

1064 You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near '0' at line 1
