Blank page using Guest Review by Perfectino addon

I installed the Guest Reviews by Perfectino

I get a blank page when I click the button,

"write a review"

I want to write reviews about the site without products involved.
Is there a add-on for this?

If you figure it out please post up the info. I get a lot of reviews via email that I would love to post for customers to see. Many of the clients don't want to log in to just post a review, giving it a bias view in my opinion.

I'm not seeing that button, but are there any error log files in your /cache folder ?
