Ok I just finished uploaded the includes (and all contents) folder only, using Fetch.
It all finished fine but there is no extra modules that have been added...
and.. my contact us page has been completely wiped :-(
How could this have happened? Will this wipe things every time I want to upload a module?

I just installed this to my test site using the method I described, and three new payment options appeared in Admin - Modules - Payment : Direct Bank Deposit AUS, NZ and USA.
If they aren't there in your admin, you've probably dropped the 'includes' folder from the mod in the wrong place. The right place is the root directory of your store (the same level that contains the folders named cache, docs, download, editors, etc.).
I have no idea about your Contact Us page. A link to your site might help.

Did you download the module from the link I gave?
I did as you said initially, and dragged and dropped the includes folder to the root level of the site. I attached an image of the FTP so you can see where I dropped it.
I also attached a pic of the modules I have after the drag and drop.
Are you able to identify the files that I may be missing, so I can look for them?
It just doesn't make sense to me.

Also, the site is applecarebox.com. not sure what happened to the contact us page but I can fix that easily enough.
is dragging and dropping the entire site off the FTP the easiest way to back up the site? thanks :-)

That looks right. Check in includes/languages/english/modules/payment for these files:
dirbankaus.php
dirbanknz.php
dirbankusa.php
and files with the same names (though they aren't the same files) in includes/modules/payment.
If both sets of files aren't there, there must be something wrong with your FTP capability.
right! The files are definitely not there. Please see my screen shot here. This is a screen of the downloaded zip package on my mac.
