Shipping Estimator for Non-Logged In Customers

On our site, when NON-logged in customers attempt to estimate their shipping costs by designating their "country" , their "state/province" and their "post/zip code", the feature does not work.

If a customer is logged in, the shipping estimator works, but when they are NOT logged in it does not work.

How can I fix this?

Store Status?
Customer Approval Status?
Zone set in shipping?

Our default shipping method is USPS, but we also offer UPS. I am not sure what you are asking when you say "Customer Approval Status". The zone set in our shipping module is "none".
