After I read this post, I had an individual email me with the problem.
He was having the same issue. So after going through a series of steps with him I found that his issue was fixed by simply realizing he didn't install this module through the admin panel.
I believe it directs you to do this in the read me notes
But just in case here you go.
Make sure you go to your zen admin-->modules-->order total and click to install this module. Then it should work as expected.

If you are using a custom template it could possibly be template specific problems. I.E did not merge files correctly.
I have run this on older versions and newer version and when installed correctly, I have had no problems whatsoever. I can't re-create the problem you are having other than by keeping it uninstalled in the back end![]()
