Blank page on checkout

I am using 1.3.9. I have added the UPSXML shipping module to it. i have one other sites where I added the module and it appears to be working fine. The customer can add items to their cart, but when they click on checkout, they get a blank page. At the blank page the URL shown is: http://fitness19store.tprintusa.net/...ckout_shipping

This is probably something dumb, but I am lost where to look. Please help.

Found the problem. I did not have Country of origin entered. Interesting consequence.

Thanks for posting the solution. It's amazing how many times the "same problem" winds up with a different fix.
