I had the same thing, more or less in my store. I had the country in 2 different shipping zones, which were then used also for the tax zone definitions.

Pretty much, if it adds VAT twice, it is because you are telling it to somewhere or other as it only does what you tell it.
Drove me mad, but I managed to find it in the end.
