Creating contact forms?

Hi, I have 3 questions:

1. How to create a similar for like this using ez pages http://lacelocswigs.com/customer_pic_submission.html

2. how to get the "browse" into ez pages?

3. how to get the "browse" as an attribute?

They are using MACHFORM by Appnitro.

We use it too.

It is an extremely simple WYSIWYG form builder, and you embed the forms into your ezpage via an <iframe>.

The form contents, when submitted are not saved in your ZC database. Machform requires its own database, but no matter... It does a neat job, and we find it a very, very useful feature - especially for clients who have no HTML knowledge... Building and embedding forms is very easy using this program.
