Captcha alternative?

As an alternative to the usual blurry captcha image, my client would like to add a field to the customer sign up form that asks a question and upon submittal, that field is validated for a specific answer.

Like the math question captcha.

Can this be done relatively easily?

If anyone has found a solution for this, this would be ideal. We are getting lots of spam registrations but do not want a traditional CAPTCHA as it would lock out our blind customers. A math/question CAPTCHA would be ideal or even something like enter the current 4-digit year.
