Does it do this when switched to classic template? In your template directory /templates is the contact TPL there? Try renaming it, probably an old template.

Switch it to classic and try your form.....

I have the same problem. I did replace the template file. It did not make any difference. Any suggestions?

This is still happening.
I have version 1.5.0 and am getting the same 3 error messages.
I have switched to another template but it was happening on the classic install..
Cannot find a solution anywhere.
