I use table rate by weight and the last cost in the list is 999.99. Customers then call to say "what the heck? $999.99 to ship something?", where we then quote them the correct price. Of course, at that point we cannot modify the order itself.
Perhaps code could be easily added that when you exceed "X" items or some weight, "CONTACT US AT 1-xxx-xxx-xxxx FOR SHIPPING RATE" is displayed and a box for the customer to add the cost quoted to them to the order. If the customer is no longer online, the store owner would be able to log in as customer (if that module was installed) and complete the transaction.
The OP asked for a simplier process than this though.
