Contact Us email not sending

Similar problem - SEND does nothing - hover over SEND and there is no URL pop-up (any other Zen Cart site I try shows the URL with the email action=send.

Have looked at (and replaced) tpl_contact_us_default.php and contact_us.php.

Have tried the dropdown list populated and empty (currently empty).

All other emails working and showing in the archive.

Any suggestions greatly appreciated.

Tried reverting to Classic Template - site does not load - had to change Template back to Customised via phpadmin (seperate problem).

All other emails working and showing in the archive.

Any suggestions greatly appreciated.
