Adding Tell-A-Friend to EZ Page

I am looking to add the Tell-A-Friend link to an EZ Page. I am using v.1.3.9h, have added a bookstore template (modified slightly), I have done a search of the community and tried the FAQ pages.

So, my questions are (in the order I think of them, not specifically in their importance):
1. Where is the Tell-A-Friend module?
2. Has this been done in the past?
3. Is there an add-on that will allow me to add it to the ezpages?
4. Any idea what else I should be asking?

Are you just wanting to add a link to the ez-page or have the content of the tell_a_friend page to appear on the ez_page?

I want to add the link to the registration page. I would also like to be able to add the referrer's email to a database field if it isn't already being added somewhere.
