import picture/displaying

Hello,

I am using Zen-Cart v1.3.9h

I like to sell T-shirts with a picture either uploaded or choosen from the site and add some text to it see attachment below , after uploading image and/or text i like to display the result and ask the customer to check an checkbox to confirm.

Someone who can tell me how to do that or is there an add on for zen cart available

I'm not sure about the checkbox, but the rest would be done with attributes:
