UPDATE!!
I have found my issue and maybe others who have posted this issue.
If you are running Dual Pricing there is something wrong with the attributes_controller.php code. I renamed my attributes_controller.php that was updated with the code for Dual Pricing using WinMerge to attributes_controller_ORIGINAL.php and loaded that up with winmerge and compared it to the actual original copy of attributes_controller.php that came with 1.3.9h and I noticed several line changes with a "w" which meant wholesale. I uploaded the original attributes_controller.php file that came with 1.3.9h to my site and now the attributes are working fine. Not sure what damage it is going to do with all my wholesale prices I have added to the products, but I guess there are other work arounds. Haven't fully tested those yet. I don't use the attributes for pricing, just sizes and stuff.
I did search this issue on the Dual Pricing Support thread and seems alot of people are having this issue with no one coming up with a resolution for the problem.
At least it appears you are getting answers. I posted about an attribute issue a couple days ago and no answers. I will review this answer.
I use the attributes controller only to make a form for customers to fill out specific info for my card shop. I can't seem to get this issue resolved. Every time I try to add an attribute and click insert it does nothing but take me back to the attribute controller page and does not insert the attribute. I tried replacing my old attribute_controller.php file from before I updated, which I think I did a couple days ago. Still didn't work. Pretty much rendering my business useless right now.
Could you look at post #6 and see if it applies to you:
