charity selection at check-out

I have browsed several forums but could not find an answer to my question. I would like to integrate a charity choice at the checkout section. The price is already in the item, but the customer can choose out of 4 charities which one he/she wants to donate to.
I need a simple radio button which will give me the additional information.

I had a look at it, it seems a handy tool and I will keep it in mind for the future, for now I might include it in to the product page, due to a lack of time. Not very elegant, but It should do the job.

If someone has another solution, please let me know.

There is also the ultimate cross sell. Cross sells appear under the cart, and if the products cross-sold don't have attributes, they can have an add to cart button integrated also. perhaps this might work also.

I just coded some functionality to include (exactly) three charity product item types, (i.e. related to three distinct product ids) into the shipping page during checkout. A quantity box is given for each charity product, so, one can purchase a couple of charity product items per type. It uses the event system so beside the checkout_shipping page in the override template no other file needs changing. I need to clean the code up when i find time, and could then offer it as a mod.

I think it would would be fairly easy to add or remove additional charity products, and with some tweaking change it to use radio buttons.
