Attributes Quantity Box ... Need Help

I need to show Quantity Boxes next to Product Attributes, like:

I had tried the following plugin, but no success:

I've already tried this plugin ... and this only works for one attribute ... I'm new to zen-cart ... and there is no help or configuration material available for this Add-On.
