How to add Join Mailing list?

I need to following:

- A side box with a button that takes the user to a page.
- User is to add his/her name, Email


I tried using the default "subscribe to newsletter" and also "join email list" add-on. Both of those are missing above requirements.

Since this is simple, would I be able to create it my self? Any add-on's that are closer to this?

I setup phplist and integrated it in to my site. I don't have it in a sidebox, but rather as an EZ-Pages link, but it would be easy enough to do in a sidebox.

I tried this. This does what I want ALMOST. I need two other fields for name and phone number.

If you really need those fields (and your customers are going to be willing to fill them in), then you will have to add them to that mod, since nothing else is as close. If you can't do it, you can hire someone.
