There is also /includes/templates/your_template/templates/tpl_modules_additional_images.php. Do you have that?

Then there is custom code in it. Rename it from .php to .bak and see what happens to the source output. (Same effect as deleting it, good for testing purposes.)

But if i removed :/includes/templates/template_default/templates/tpl_modules_additional_images.php then it breaks the page

Then check (compare) that and the /template_default/ version of it against a fresh copy. I think that's the last file that could be involved in making your custom output, so it has to be that or one of the other files you identified.
