I'm getting something similar - missing billing addresses and also in my address book. All there in the database. I get this is 1.3.9 and testing 1.5.0RC2 on MAMP.

Finally nailed this sucker. It was the address_format table only having formats 1 to 5 and so I just sqled 6 in and everything is working.

That suggests you missed important upgrade steps and thus could have numerous other serious data problems in your database.
