I have been working on a store for a not for profit organization. They are looking to collect membership dues through the store, but with it must come a membership form. I have been searching the forums but am not sure if there is a way to ask for additional information when purchasing an item? (Obviously name, address, email will come through with the order, but I need a few more items as well).
Right now they take all membership applications through snail mail with check or money order and a printed PDF form.
I apologize in advance if I overlooked a thread with this information.
