Missing Phone Number

Ok...I have done something that made the phone number part of the form disappear when someone is creating a new account. The fax number area is there, but not my phone number box where a new customer can put their phone number.

I checked in the admin side to see what it was set on and its on 3, so that isn't the issue. Can someone tell me what file I need to look at to get the phone number back into the form?

I have no idea what add-on done this, so I need to find out.
I feel like a dunce with this one...lol
