How can i Customize my header of the template i want to remove "log in" from the header and i want to insert a simple form with "User Name" & "Passwor" so that customer can log-in directly instead of first clicking on "log-in" link.

Are you actually still using version 1.2 of the cart? If so, you REALLY need to consider upgrading.

And don't ever publicize the site name before you upgrade, or you will be hacked as soon as a hacker sees it.
