Is it possible to deny shipments to PO Boxes? and surcharge AK+HI shipments?

We don't want to ship to PO Boxes because I would have to use the USPS and who would want to do that? :)

That is my main question but it would also be nice if I could upcharge shipping to AK & HI shipments or if I could just not allow shipments to AK & HI altogether. I'm not sure what to do.

There's the PO Box Ban Add-On; I haven't used it ...

I believe that you could set up one zone for the continental US and another zone for AK+HI, setting a shipping surcharge for the AK+HI zone.
