How do I create a custom form?

hi folks, just wondering if there's a straight forward and easy way of creating a custom form in zen cart. it's not a complicated form, about six to seven fields e.g name, telephone number etc and a submit button with a code that submits the information, just like the contact us page.

You can put an HTML form in an EZ page. We use Machforms - not free, but cheap and does a great job with an easy-to-use admin.
