Trouble with UPS module

After installing UPS module, I am getting this every time I check out:

We are unable to obtain a rate quote for UPS shipping.
Please contact the store if no other alternative is shown.

The only change I've made was setting pickup mode to OTP.
What am I doing wrong?

What is the weight of the order?

What is the Ship From?

What is the Ship to?

Larger packages - added packaging percentage:weight 10:0

Order weight is about 2lb.
Ship from is our shop address in Canada, Ship to is the customer's shop address in CA, USA. I can get a quote using both on UPS.com

The UPS that comes with Zen Cart requires a 5 digit US Zip Code ...

For other countries, look in the Free Software Add Ons for the UPS XML ...

Are you referring to the sender's address?
Receiver's California address has a 5 digit US zip code.

It would be helpful if it was mentioned in the module, that it was only for the US stores. I've spent hours trying to figure why it just refused to work.
