No that didn't work either. My template didn't have tpl_index_categories.php, it is in the template_default, so I added your tpl_index_categories.php to my template and it still didn't work with or without the categories set to true or off. I also added the tpl_index_categories.php from the template_default, it didn't work, Must be something else, Thanks any way for your help.
