I determined what was wrong and can reproduce the issue as well as the fix:

The formatting in the Set "Contact Us" Email Dropdown List did not follow the clearly defined instructions in Configuration | E-Mail Options. My supposition above that it stopped working is likely wrong; my best guess now is that it never worked as I only listed the email addresses I wanted in the drop-down, not the aliases and addresses as appears to be required.
