Adding Notes to Per Ordered Item

Hi.
I have a requirement for customers to add notes/comments to each Ordered Item, as the customers could be ordering 30+ different items on one order, they want to be able to add specific detail for that Item.

Does anyone know if there is any current Add-in that can do this.
(using Order Comments 'Single comment per Order' is not good enough for our requirements)

You can always use the attributes controller to add a text box to the product.
