THAT is the module you need to install. And it will present the credit card fields automatically for you. Your boss's form/design is unnecessary.

The module you need to install is whatever module is required by the company you have hired to do your credit card processing. Hence the first question I asked, which you didn't answer.

To accept payments via PayPal, you simply enable and configure the built-in PayPal Express Checkout module.
PayPal allows your customers to pay by credit card, but ALL those credit card details are entered on the PayPal site.

IF YOU ALSO WANT to accept credit card payments directly on your website, you need to have a Merchant Account with a gateway which provides credit card processing services. And you must have SSL enabled on your site. And your site must pass PCI compliance and everything you customize in the code is also your responsibility to ensure is PCI compliant. And from my observations you have been making HUGE changes. I hope you have had security in mind while you've been changing things.

Once you have established a contract with a payment gateway, they will tell you which technology/module/method you must use on your website to collect payments. At that point you will need to install whatever addon payment module is designed to work with your payment gateway company. And that module will display credit card input fields at checkout for you. YOU WILL NOT CUSTOMIZE OR DESIGN ANYTHING FOR CREDIT CARD PAYMENT FIELDS until you have the gateway and the module working.
