Tell a Friend Sidebox not displaying

I recently upgraded to 1.50 and for some reason the Tell A Friend sidebox won't display.

I think I have it setup correctly; in Layout Boxes Controller, I have:

LEFT/RIGHT STATUS: ON
LEFT OR RIGHT: RIGHT
LEFT / TIGHT SORT: 250 (nothing else has this sort number)
SINGLE SORT: 0 (template is left and right columns)
SINGLE STATUS: OFF

The box above it in the controller is showing but it doesn't show the Tell a Friend box.

I've tried change the sort order but no luck.

Any idea what may be causing this?

Oh Doh!

It only shows on Product pages of course - stupid me!

But - it is showing weird - as in the button seems missing - see image



Where do I look to correct it?

Looks like the button image isn't where Zencart is looking for it.
