Users have to log in twice

Hello,
I'm completely new to zen cart (but do have experience with PHP,MySQL). I've been brought in on a problem one of my clients has noticed -- that their users have to log in twice. The first time, it just reloads the page. The second time you try, it logs the user in (also, as expected, if the user tries logging in in the next few minutes it works. If I clear my cache, the issue arises again and I once again have to log in twice).

The site uses Zen cart version is 1.3.8 -- I would imagine that I'll need to upgrade this at some point here, but I'm hoping I can fix this first issue quickly, and then work through the larger tasks of upgrading. I'm not entirely sure how long the issue has been present, in January the server was upgraded from PHP 5.3.3 to 5.3.6, so I'm not sure if it could have happened then.

Also, other possible insights: for this site, they have to approve people before they can order. This sits on a subdomain of the main site, where they don't have to approve people before ordering. The issue only seems to present itself on the site that they have to approve members for.

When the user creates an account, does he see the Customer Authorization Pending ... page ?

What are the settings in Admin - Configuration - Customer Details - Customer Shop Status - View Shop and Prices and Customer Approval Status - Authorization Pending ?

Yes, please upgrade to at least 1.3.9h ASAP.

The 'Customer Authorization Pending' page looks like it displays correctly to the user.
The settings in the Admin Configuration are:
Customer Shop Status - View Shop and Prices: 0
Customer Approval Status - Authorization Pending: 2

Looking into the upgrade procedures currently...
