Thank you David Allen that was what I was looking for.
It is nothing in the template it is clearly the functionality of the button/ form fields that has a problem. I just need to find a previous version of the contact_us header and hopefully figure what has gone wrong.

Is this the only place that could be a problem?
Is there any other place(maybe on the actual zen-cart control panel) that you could modify that? (I am just wondering how can this have gotten so messed up, thinking that maybe someone that has access there could have broken it)

Also not sure if this matters but the email has a template as well, with an image. So when it is sent to the contact email address it appears as from Name (from the field)
and then there is a template where the message is in. But no email address or any other info. Is there somewhere I can check this template?

The email template is in /emails/email_template_contact_us.html That probably hasn't changed too much - the image is in the same folder. It is the header_php.php file that sets up the data that is slotted into the email template.
