Looking for Tutorial Using the Zone Table

I am thinking about using the Zone table for my flat rate shipping....but am at a loss on all of the zone ISo country codes. Really, what I am looking for is the US having a single zone with a sing flat rate, based on price, I guess that would be something like:

500:4.99 (assuming this means anything UNDER $500.00 will be $4.99)

er, what if someone buys $501.00 worth of merchandise? What happens?

For the rest of the world, do I need to enter EVERY single ISO Country Code?

I assume, for the world I would have something like the following:

500:9.99 (assuming this means anything UNDER $500.00 will be $4.99)

Am I on the right track here on how to handle different pricing depending on the country?

At 500.01 the rate will be $0.00 ...

For ALL countries outside the US, you can use:
00

NOTE: Make sure that when using 00 for ALL Zones not otherwise defined that you use it as the LAST Zone the you define ...

On the Rates, this is a separate definition for each Zone you define and the rates are not related ...
