Attributes will not insert into a product

Hi, I have added numerous option names, i.e. Name, Address, Tele, ect. I have numerous products on my site already and would like to add attributes so that a form can be filled out and a photo uploaded when ordering one of my cards. I am not real new to this. I have set up Zen Carts before and added attributes to products just fine. Here is what I am doing.

1. I set up option names for all of my fields, which are all text and upload so I didn't create any option values.

2. I went to the Attributes Controller in Catalog

3. I choose the product I wanted to add the attribute to from the categories section of the attributes controller and clicked "display"

4. I go down to the "Adding New Attributes" section, click on an attribute I want to add and click insert. It does not add the attribute. What I see after I insert, is what I have attached here for your review.

Please help. I need to get the attributes on my invitations before I can sell them.

If you click on the Option Name:
City

what do you see in the right section for Option Value?


If you see the Option Value:
TEXT/UPLOAD FILE [#0]

and now, if you click on the Option Name:
City

and then on the Option Value:
TEXT/UPLOAD FILE [#0]

and now try to add the Attribute, what happens?

If I click on the Option Name: City, I do see in the Option value TEXT/UPLOAD FILE [#0], the second option, worked. Thank you so much.

It is odd you have to do it that way.

You need to select a pair for the Option Name and Option Value ...

For TEXT Attributes, that is the pair ...

What did you mean when you said:
