Duplicate entry for key insert into sessions

I hope somebody can help me figure out what is going on.

I am getting the fatal error below. The store has been running for a couple of months, but I just I started seeing this error last week (twice) after I was prompted in admin to clear the activity logs and cleared them, but I don't see how this problem could be related. I also added images to the store and updated the products using EasyPopulate.

Any help would be greatly appreciated.

I am running Zen Cart 1.5.1, with UPS shipping module, Sage Payment Module, TaxCloud.

Ask your host what changes they've made to PHP versions or PHP configuration settings lately.

And, go to Admin->Tools->Server Info and post everything from that page here, mainly the PHPInfo settings.

What sections of the PHP.INI file should I post? For security reasons, I don't want to post all the settings, or I could edit paths, IPs, domain, ports.

Yours probably has:
session.hash_function = sha512

but changing it to the original setting of 1 should avoid that issue:
session.hash_function = 1

or you could try the even older setting of 0 .... both will work fine with ZC

It was set to 0. The session settings are included below. I set it to session.hash_function = 1 in PHP.INI

You probably know this, but I looked it up and include it here for others that may want to know '0' means MD5 (128 bits) and '1' means SHA-1 (160 bits).

When made the change, it was very slow to login, and I couldn't get a UPS shipping quote returned from UPS. Maybe something to do with creating a session? When I tried set it back to 0, the UPS shipping quote worked again. When I tried this I hadn't been previously logged in, so there wasn't a stored session, and I did try closing my browser and logging in to the store again too a few times.

0 is an acceptable setting also. (to clarify, I've updated my previous post to reflect that.)

Have you been able to establish any pattern of steps which specifically causes the error to be triggered?
