Tell a Friend Mod / Problem Sending

I did a test on the Tell a Friend Mod and this is the error code I get:

Whoops! The page could not be found. Try giving it another chance below.

Anyone know what is wrong and how to fix this? Here is the product page where the Tell a Friend is:

If using version 1.5.0 - tell a friend was removed
You apparently did not upgrade your template files accordingly

Right, that's why I installed the Mod for it.. I'm not sure what you mean by "upgrade your template files" ? Maybe this mod doesn't work with my version?

The only plugins I see for this are for ZenCart V1.3.8 - So most likely they rely upon core code that was removed
