Hello,
I believe I messed up something, recovering from a Malware infest from this past weekend, fixed some files, and now my admin login screen has only this..
blank with this in the center ( <form id="loginForm" name="loginForm" action=" )
Did I under/over look something or load some in-correctly?
Any insight on this would help...

Sounds like you've still got damaged files.
I'd be replacing them from backups..
