Option Values Manager in admin: duplicate entries for each item...why?

I had an issue with the Attributes Controller throwing an error just saying "WARNING: An Error occurred, please refresh the page and try again". I found a thread and figured out that the first line in the products_options_values table didn't have the data "TEXT" under products_options_values_name! This was causing that error. So I fixed that but now when I go into the Option Values Manager it's showing each entry twice! When I checked the database however with phpMyAdmin it indeed properly only has one entry per id... so it's the Manager that's showing duplicates incorrectly.

It's a major annoyance but I am also scared something is wrong. Anyone know what the issue could be?



I've attached the image above to show you what I am talking about. It's a stock Zen v1.51 installed (from new) with imageHandler4 being the only add-on installed.
